Creating links to attachments doubles/+ entries in the attachment list

We are trying to add attachments to a file and replace the hyperlinks in the primary document to launch the attachments. When we add the annotations, we get multiple entries in the attachment list. The desired result should be only one entry in the attachment list (also, it appears that the extra entries are increasing the size of the final document. I’ve uploaded a test project that reproduces these results along with test data. We are currently using version 19.2.
