I also have encountered this error - The original developer created a word document after installing the library on a development server - I’ve deployed our solution that uses the library in a web service (WCF) to our PreProduction web server and no checkboxes render as small fat circles.
Any update to this would be useful.

Thanks for your request. Please make sure that Wingdings 2 font is installed on the PC where you perform conversion to PDF. If not, please try to install Wingdings2 font. This should fix the problem. Please let me know in case of any issues. I will be glad to help you.
