Creating a table of contents from an existing document with bookmarks

Hi,

I have created a pdf with bookmarks by merging streams of pdfs, and adding the bookmarks with each document. The bookmarks were straightforward, but is there a way to generate a table of contents at the same time?

If not, is there a way of to create a table of contents for an existing pdf that has bookmarks - using the bookmarks for the entries in the table of contents?

We have all the dlls.
