Creating PDF with more than 400 pages ERROR : ‘System.OutOfMemoryException’

We are trying to build a document by appending pages from one PDF to a larger file.
When the doc size gets to 400 pages or above we get an out of memory error.
Please send us code samples of the recomended method if available.
