Hyperlink a word

I need to go through an existing PDF document, locate every instance of a specific word and link that word to another document. I found an example in your documentation that shows how to create a hyperlink to a region, but how do I create a hyperlink to a word?

Along with that can I reformat the word to be a different color as an indicator that it is a link?
