StartIndex cannot be zero error

When I do an OpenInBrowser save such as:

pdf.Save(myfilename, Aspose.Pdf.SaveType.OpenInBrowser, this.Response);

I am getting a StartIndex cannot be zero error. If I just save to a disk file I don’t get the same error and I can open the resulting file with acrobat just fine.

I am creating this pdf document from scratch using code, rather than converting from a word doc. I have a title page, table of contents and then the document body text.
